护理干预对PICC置管化疗肿瘤患者并发症的影响

目的探讨护理干预措施对降低肿瘤内科PICC置管患者并发症的影响。方法回顾性分析我院肿瘤内科经PICC置管发生置管相关并发症85例患者资料,并通过开展强化护理培训、规范PICC置管操作流程、积极处理并发症、加强健康宣教等综合性护理干预措施,预防置管相关并发症的发生。结果①干预前两年间PICC置管患者并发症发生率11.14%,并发症例次构成分别为导管堵塞27.68%、穿刺点渗血21.43%、置管处感染16.96%、静脉炎13.39%、导管移位9.56%、导管脱出7.14%、导管破裂3.57%,非正常拔管构成比为18.82%。②干预后半年内PICC置管患者并发症发生率为7.07%,并发症构成分别为穿刺点渗血36.36%、导管堵塞27.27%、静脉炎22.73%、导管移位9.09%、导管脱出4.55%,非正常拔管构成比为7.14%。结论实施系统化护理干预是减少PICC置管化疗患者相关并发症发生的必要和有效措施。

Nursing Intervention and Effect on Complications of Cancer Patients with PICC Chemotherapy

Objective The paper is to observe nursing interventions and its effect on complications of cancer patients with PICC chemotherapy. Methods Respective study was conducted on 85 cancer patients with PICC related complications,based on which intensive nursing training, standardization on PICC operation process, complications treatment, strengthening health education were conducted to prevent occurrence of complications.Result （1）2 years before nursing interventions, incidence rate of PICC complications was 11.14%;Composition of complications were catheter obstruction in 27.68%, oozing of puncture point 21.43%, catheter infection 16.96%, phlebitis 13.39%, catheter displacement 9,82%, Catheter slippage 7.14 %, catheter rupture 3.57% respectively, among which l6 patients with abnormal extubation. （2）Half a year after nursing intervention,incidence rate was7.07%, incidence composition were oozing of puncture point 36.36% catheter obstruction 27.27%,phlebitis 22.73% catheter displacement 9.09%, Catheter slippage 4.55%,abnormal extubation was 7.14%.Conclusion Systematic nursing interventions are necessary and effective measures for reducing incidence of catheter complications related PICC therapy.
