异基因造血干细胞移植治疗恶性血液病的研究

目的　探讨异基因造血干细胞移植治疗恶性血液病的疗效。方法　对 1例慢性髓性白血病 (慢性期 )和 2例急性髓性白血病患者行异基因外周血干细胞移植 (Allo -PBSCT) ,对 1例多次复发的急性髓性白血病患者行非亲缘异基因骨髓移植 (URD -BMT) ,并观察疗效及不良反应。结果　 4例患者均获得造血重建 ,MACV方案及改良BU/CY方案预处理均适合于异基因造血干细胞移植 ,CsA联合MTX能有效预防移植物抗宿主病 ,单用前列腺E1脂质微球可预防肝静脉闭塞病。结论　对有合适供者的髓性白血病患者行Allo -PBSCT可获得良好疗效 ,URD -BMT可用于治疗多次复发的急性髓性白血病

Study on allogeneic hematopoietic stem cell transplantation to treat for malignant hematologic disease

Objective It is to study the curative effect of allogeneic hematopoietic stem cell transplantation on malignant hematologic disease. Methods Carrying allogeneic peripheral blood stem cell transplantation(Allo-PBSCT) on one case of chronic myeloid leukemia(chronic phase) and two cases of acute myeloid leukemia. And carrying unrelated donor allogeneic bone marrow transplantation(URD-BMT) on one case of acute myeloid leukemia which recrudesce many times as well. Then observed the treatment effect and adverse effect.Results All of the four cases can receive hematopoietic function reconstruction. The pretreatment programs of MACV and improved BU/CY are suitable for Allo-PBSCT. And the program of CsA associated with MTX can effectively prevent graft versus host disease(GVHD). Singly use Lipo prostaglandin E 1 can prevent hepatic veno-occlusive disease(VOD). Conclusion The treatment of Allo-PBSCT to patients with myeloid leukemia can receive a good effect when there is a suitable afforder. URD-BMT is suitable to treat acute myeloid leukemia which recrudesce many times.
