艾司洛尔复合硝酸甘油控制性降压在鼻内窥镜手术中的应用

目的:比较艾司洛尔复合硝酸甘油与单纯硝酸甘油控制性降压在鼻内窥镜手术中的效果。方法:将40例择期行鼻内镜手术的患者随机分为2组,试验组(n=20)患者给予静脉注射艾司洛尔复合硝酸甘油降压;对照组(n=20)患者单纯应用硝酸甘油降压。分别观察记录2组患者的平均动脉压(mean arterial blood pressure,MAP)、心率(HR)、术中失血量及手术时间。结果:试验组和对照组患者在艾司洛尔复合硝酸甘油或单纯硝酸甘油诱导后,MAP较诱导前均显著降低(P均<0.05);试验组患者诱导后HR较诱导前显著降低(P<0.05),对照组患者诱导后HR较诱导前显著升高(P<0.05)。试验组患者术中失血量、手术时间均显著少于对照组(P<0.05)。结论:艾司洛尔复合硝酸甘油应用于鼻内窥镜手术进行控制性降压,效果确切、稳定,可控性强,安全可行。

Controlled Hypotension Induced by Esmolol Hydrochloride and Nitroglycerin in Functional Endoscopic Sinus Surgery

Objective：To investigate the effect of continuous infusion of nitroglycerin with or without esmolol hydrochloride in functional endoscopic sinus surgery （FESS）. Methods： Forty patients scheduled for functional endoscopic sinus surgery were randomly divided into the study group （n = 20, nitroglycerin ＋ esmolol hydrochloride ） and the control group （n = 20, nitro- glycerin）. Data of blood pressure（BP）, heart rate（HR）, blood loss volume（BLV） and operation time（OT） were recorded. Results： The mean arterial blood pressure（MAP） in both groups were significantly decreased after induction （P〈0.05）. The average HR in the study group decreased significantly after the induction of nitroglycerin and esmolol hydrochloride（P〈0.05）, while the average HR in the control group increased significantly after the induction of nitroglycerin（P〈0.05） . The BLV and OT in the study group were significantly less than those in the control group（P〈0.05）. Conclusion： The combination of nitro- glycerin and esmolol hydrochloride can be applied in FESS to achieve exact and stable controlled hypotension.
