摘    要:目的探讨FP-7Ahmed青光眼阀植入术联合广泛视网膜光凝治疗新生血管性青光眼的临床效果。方法2008年10月至2011年8月于沧州市中心医院行FP-7Ahmed青光眼引流阀植入术联合广泛视网膜光凝的新生血管性青光眼38例(38眼)。随访6—24个月,观察眼压、手术并发症及成功率。结果患者术前眼压为(42.95±12.04)mmHg(1mm取=0.133kPa)。术后1周、1个月、6个月、12个月、24个月眼压分别为(12.82±3.78)、(14.55±4.31)、(14.91±5.01)、(16.37±4.42)、(17.71±3.22)mmHg。不同时间点的眼压比较,差异有统计学意义(F=91.58,P〈0.05)。术后各时间点的累积总成功率分别为100%、97%、92%、86%和79%。术后共有7例(18.42%)出现了并发症,其中4例于用药后恢复,3例需手术处理,无一例出现术后严重并发症。结论FP-7Ahmed青光眼引流阀植入联合广泛视网膜光凝是治疗新生血管性青光眼比较有效和相对安全的治疗方法,可作为新生血管性青光眼的首选治疗方式之一。

FP-7 Ahmed glaucoma valve implantation combined with panretinal photocoagulation for neovascular glaucoma

Abstract:Objective To evaluate the clinical efficacy of FP-7 Ahmed glaucoma valve implantation combined with panretinal photocoagulation for neovaseular glaucoma. Methods 38 eyes of 38 patients with neovascular glaucoma who underwent FP-7 Ahmed glaucoma valve implantation combined with panretinal photocoagulation were collected in this study in the Central Hospital of Cangzhou from October 2008 to August 2011. The follow-up ranged from 6 to 24 months. Changes of intraocular pressure (IOP), postoperative complications and success rate were followed up. Results Compared with preoperative IOP which was (42.95 ± 12.04) mmHg, the postoperative IOP was significantly decreased (F = 91.58, P 〈 0.05), and were (12.82±3.78), (14.55±4.31), (14.91±5.01), (16.37±4.42), and (17.71± 3.22) mmHg at week 1, month 1, month 6, month 12, and month 24 after surgery respectively. The total cumulative success rates of each time point after surgery were 100%, 97%, 92%, 86% and 79% respectively. The postoperative complications occurred in 7 eyes (lg. 4% ). 4 cases recovered after medication and 3 cases needed operation treatment. None serious postoperative complications occurred in all patients. Conclusion FP-7 Ahmed glaucoma valve implantation combined with panretinal photocoagulation may be an effective and safe method for treating patients with neovascular glaucoma, and it can be used as one of the preferred methods for management of neovascular glaucoma.
