扩大翼点开颅去骨瓣减压术治疗大面积脑梗塞分析

摘    要:目的探讨扩大翼点入路开颅去骨瓣减压术加颞肌贴敷术治疗大面积脑梗塞的手术适应症、手术方法及治疗效果。方法对我科2006年2月-2009年8月对18N大面积脑梗塞行扩大翼点入路开颅去骨瓣减压术加颞肌贴敷术治疗的患者进行回顾性分析研究,以GOs评分评定效果。结果术后随访3~6个月,18倒中16例生存,2例死亡(11.1%),植物生存1例(5.6%),重残5例(27.8%),中残7~q(38.9%),良好3例(16.7%)。结论扩大翼点入路开颅去骨瓣减压术加颞肌贴教术治疗大面积脑梗塞效果良好,手术时机的选择至关重要。

Abstract:Objective To explore the indication for surgical, operation Method and the effect about amplification of pterion approach decompressive surgery and roofing of temporal muscle in the treatment of massive cerebral infarction. Methods The clinical data of 18 patients with massive cerebral infarction which treated by amplification of pterion approach decompressive surgery and roofing of temporal muscle from February, 2006 to August, 2009, were analyzed retrospectively. Results According to Glasgow outcome scale, there was died in 2 patients (11.1%), vegetative survival in 1 (5.6%), severe disability in 5 (27.8%), moderate disability in 7 (38.9%),good recovery in 3 (16.7%). Conclusion Amplification ofpterion approach decompressive surgery and roofing of temporal muscle is profitable for patient' s with massive cerebral infarction, time of surgery is important.
