两种剂量辛伐他汀治疗急性冠脉综合征的临床疗效评价

目的:研究短期大剂量(40mg/d)和低剂量(20mg/d)辛伐他汀对急性冠脉综合征(ACS)治疗l周后血清高敏C反应蛋白(hs-CRP)、白细胞介素6(IL-6)、假性血友病因子(vWF)及血脂水平的影响.方法:对临床确诊为ACS的69例患者随机分入A组(23例)、B组(23例)和C组(23例),分别接受常规治疗(不给予任何调脂药)、辛伐他汀20mg/d和40mg/d治疗1周,同时测量各组治疗前后hs-CRP、IL-6、vWF和血脂水平的变化.结果:C组hs-CRP、IL-6、vWF治疗l周后差值与A组、B组治疗前后差值有显著意义(P＜0.001);治疗前后以上指标自身比较在C组差异有显著意义(P＜0.05).血脂水平治疗前后差异无显著意义(P＞0.05).结论:短期辛伐他汀强化治疗,可明显减少炎症反应,稳定斑块,改善血管内皮功能.

Evaluation of the Clinical Effect of Two Different Dosage of Simvastatin on Acute Coronary Syndrome

OBJECTIVE: To study the effect of two different dosage of simvastatin(large dosage,40mg/d and low dosage,20mg/d for one week) on hs-CRP, IL-6,vWF and lipids in patients with acute coronary syndrome(ACS). METHODS: 69 patients with ACS were randomly divided into three groups: group A (23 cases),group B(23 cases)and group C(23 cases).They were respectively given 20mg/d and 40mg/d simvastatin for one week without lipid-lowering drugs. Plasma levels of hs-CRP, IL-6,vWF and lipid in each group before and after treatment were detected. RESULTS: The levels of hs-CRP, IL-6 and vWF significantly decreased in group C than those in group A and group B after one week's treatment (P<0.05). The levels of TC, TG,HDL-C and LDL-C had no change in three groups after treatment (P>0.05).CONCLUSION: For the patients with ACS, using 40mg/d simvastatin in a short period can obviously decrease inflammatory reaction, stabilize the plaque and improve blood vessel endothelium function.
