肝原发性软组织恶性巨细胞瘤临床病理观察

目的 初步探讨肝原发性软组织恶性巨细胞瘤的临床病理学特点、免疫组化染色及患者预后。方法 对1例原发性软组织恶性巨细胞瘤标本常规石蜡切片，HE染色，光镜观察；另对石蜡切片采用EnVision免疫组化二步法染色。结果镜下见瘤组织呈多结节状，结节由多核破骨细胞样巨细胞、单核瘤细胞和少量纤维母细胞所构成。免疫组化染色见瘤细胞vimentin和CD68（＋）。结论 本例肿瘤的瘤细胞虽然无明显的异型性，但患者于术后3个月死亡，说明仅凭形态学改变并不能可靠地预测肿瘤的生物学行为，尽管肿瘤在形态学上并无明显的恶性指征，肿瘤也可为高度恶性。

Primary malignant giant cell tumor of soft tissue in liver: a clinicopathological observation

Objective To explore the clinicopathologic features,immunohistochemical staining and the prognosis in a rare case of primary malignant giant cell tumor in liver.Methods The tumor was studied by using paraffin slices,HE staining,immunohistochemical method(EnVision method) and observed under microscope.Results Under microscope,the tumor showed the nodular structures which were consisted of osteoclast-like multinuclear giant cells,mononuclear tumor cells and fibroblasts.Immunochemical stain revelaed that the tumor cells were reactive with antibodies of vimentin and CD68 etc.Conclusion Although the tumor cells do not show remarkable atypical morphological appearance,the patient died 3 months after the operation.It is suggested that we could not predict the biological behavior of the tumor by observing the morphological appearance under microscope alone.In this case,great size of the tumor,quantity of nuclear mitoses,necrosis in some area in the tumor and infiltration of the tumor cell into hapatocytes in focus area indicate that the tumor is malignant.
