老年男性高血压与前列腺增生的相关性研究

目的探讨高血压与良性前列腺增生发生、发展的相关性。方法选择男性前列腺增生患者223例,根据欧洲高血压学会/欧洲心脏学会高血压治疗指南将患者分为血压正常组(正常组)66例和原发性高血压组(高血压组)157例。其中高血压组病程≤15年患者96例,>15年患者61例;杓型血压患者58例和非杓型血压患者99例。检测血压、血脂和前列腺特异性抗原(PSA)等;超声测量并计算前列腺体积;行24h动态血压监测。结果与正常组比较,高血压组全天平均收缩压、前列腺体积、前列腺体积年增长率明显升高(P<0.05)。前列腺体积与高血压病程和PSA呈正相关(r=0.18、r=0.56,P<0.05),前列腺体积年增长率与年龄、PSA及体质量指数呈正相关(r=-0.20、r=0.49、r=0.20,P<0.05)。高血压病程>15年及非杓型血压患者前列腺体积及前列腺体积年增长率明显高于高血压病程≤15年及杓型血压患者(P<0.05)。结论前列腺增生发生、发展与高血压密切相关。高血压病程及其血压昼夜节律对良性前列腺增生均有影响。

Correlation between hypertension and prostatic hyperplasia in elderly male hypertensive patients

Objective To study the correlation between hypertension and benign prostatic hyperplasia(BPH).Methods Two hundred and twenty-three male BPH patients were divided into normotensive group(n=66)and primary hypertension group(n=157)according to the ESH/ESC Hypertension Treatment Guidlines.The course of hypertension was ≤15years in 96patients and>15years in 61patients of primary hypertension group.Dipping hypertension was diagnosed in 58patients and non-dipping hypertension was diagnosed in 99patients of primary hypertension group.Their blood pressure,blood lipid and PSA were measured,their prostate volume(PV)was calculated,and their 24hambulatory blood pressure was monitored.Results The mean SBP,PV and annual prostate growth were significantly higher in primary hypertension group than in normotensive group(P<0.05).The PV was positively related with the course of hypertension and PSA(r=0.18,r=0.56,P<0.05),and the annual prostate growth was positively related with the age,PSA and BMI(r=-0.20,r=0.49,r=0.20,P<0.05).The PV and annual prostate growth were significantly higher in non-dipping hypertensive patients with their course of hypertension>15 years than in dipping hypertensive patients with their course of hypertension ≤15years(P< 0.05).Conclusion Prostatic hyperplasia is closely related with hypertension.The course of hypertension and its circadian rhythm can exert their effect on BPH.
