彩色多普勒超声诊断肝外型门静脉高压症的临床价值

目的：探讨肝外型门静脉高压症（PHS）彩色多普勒超声诊断价值与鉴别诊断。方法：回顾性分析了39例经血管造影（DSA）证实并行手术或介入治疗的肝外型PHS声像图表现。按肝外型PHS病变解剖部位分：肝前型PHS22例，肝后型PHS17例。结果：1、超声定位正确诊断率100％；2、肝前型PHS临床以食管胃底静脉曲张或破裂出血为首发症状，说明食管胃底静脉曲线的程度与肝功能状态无关，而与PHS类型有关；3、肝后型PHS以肝脾早期明显肿大为主。结论：彩色多普勒超声对肝外型PHS的定位诊断有着重要的临床治疗价值。

Clinic Value of the Out-hepatic Portal Hypertension Syndrome Diagnosed by Color Doppler Ultrasound

Objective:To evaluate the value of color Doppler ultrasound diagnosis and differential diagnosis in out hepatic portal hypertension syndrome(PHS).Methods:This paper retrospectively analysed out hepatic PHS image features in 39 cases confirmed by DSA,operation or interventional therapy.According to the anatomic and pathological changes,out hepatic PHS could be classified into 22 cases with pre hepatic PHS and 17 cases with post hepatic PHS.Results:The accuracy rate of ultrasound diagnosis for location was 100%.Clinically,the pre hepatic PHS patient attacked first with esophageal and gastric fundus varices or bleeding.The esophageal and gastric fundus variceal degree was not directly related to the state of the hepatic function,but related to the type of PHS.The post hepatic PHS patient mainly appeared with early hepatic splenic swelling.Conclusions:The color Doppler ultrasound has provided for diagnosing the types of out hepatic PHS and has displayed its significant clinic values in the treatment.
