胸部数字化摄片在肺癌筛查中的价值

目的分析胸部数字化摄片（Digitalradiography，DR）对高危人群早期肺癌筛查的价值。方法对2007年1月~2007年12月四川大学华西医院健康体检中心参加体检人群中的肺癌高危人群进行胸部数字化摄片检查并分析。共有14452人（男性9124人，女性5328人）纳入研究，年龄40～92岁，平均54岁，其中有吸烟史达到标准的2297人（男性2291人，女性6人）。将人群分为甲组（吸烟组）和乙组（不吸烟组）。结果在两组人群中DR发现纤维条索影的发现率的差异有统计学意义。筛检出肺癌6例，其中5例为单结节影，1例为多结节影。6例肺癌中3例只有1个危险因素，3例有2个危险因素，危险因素多少不同，肺癌发生率上的差异有统计学意义。结论胸部DR能发现大部分胸部病变，对有吸烟危险因素的肺癌高危人群进行胸部DR筛查是必要的。

Value of Chest Digital Radiography in Screening for Lung Cancer

Objective To analyse the value of chest digital radiography（DR） in early screening for lung cancer in high-risk groups. Methods Chest DR were applied for people who took physical examinations in health center of West China Hospital of Sichuan University during Jan. 2007 to Dec. 2007, and the results were analysed carefully. There were 14452 cases （9124 males and 5328 females） in the study, aged 40 to 92 years old, and averaged at 54. Among all the people there were 2297 smokers, who were classified into group A, and the rest were group B. Results The detected ratio of patchy opacities and mediastinal lesions in the two groups by chest DR had not（P〉0.05）, while that of linear opacity, nodules and pleural lesions did have significant difference（P〈0. 05）. Besides, we screened out six lung cancer patients. One was present as multiple nodules, and the others was single nodule. Further more, three cases in the six lung cancer patients had only one risk factor, and the rest had two. There was significant difference of lung cancer incidence among the groups with different numbers of risk factors, which meant the more risk factors, the greater chance of lung cancer. Conclusion Chest DR had the capacity to detect most of the lesions, and it was necessary for the high-risk groups, such as smokers, to take chest DR examinations.
