剖宫产术同时行子宫肌瘤剔除451例分析

目的探讨对妊娠合并子宫肌瘤患者在剖宫产术中同时行肌瘤剔除术的可行性和安全性。方法回顾性分析2009年10月至2011年9月我院妊娠合并子宫肌瘤451例在剖宫产术中同时行肌瘤剔除术患者的临床资料,选择同期无任何合并症的正常剖宫产患者451例为对照组,统计两组在术中出血量、术后24h出血量、手术前后血红蛋白差值、子宫复旧情况差异。结果观察组与对照组相比,手术时间、肠蠕动恢复时间延长,差异有统计学意义(P<0.05),术中出血量及住院时间等比较差异无统计学意义(P>0.05)。结论在病例选择合适、手术方法得当的情况下,剖宫产术中同时行肌瘤剔除术是安全可行的。

Clinical Analysis of 451 Cases Hysterectomy during Cesarean Section

Objective To study the safety and feasibility of hysterectomy in pregnant women with hysteromyoma during cesarean section. Methods We retrospectively analysed 451 cases of pregnant women with hysteromyoma, from October 1 st, 2009 to September 30th, 2011, who received hysterectomy in the process of cesarean section in our hospital （the observation group）, which were compared with 451 cases of pregnant women with simple cesarean section （control group）.The blood loss during the operations, blood loss after 24 hours, postoperative hemoglobin difference, differences in uterine involution and so on were compared between the observation group and control group. Results Between the observation group and control group, the former had the extended operation time and postoperative anal exhaust time, and the difference was statistically significant （P〈0.05）, however, blood loss, the length of stay and so on were not statistically significant （P〉0.05）. Conclusion For the proper patients and with appropriate surgical technique, it is safe and feasible to perform myomectomy during cesarean section.
