瑞芬太尼复合丙泊酚用于精神障碍孕妇人流术的临床观察

目的观察瑞芬太尼复合丙泊酚用于精神障碍孕妇无痛人流术的安全性及适用性。方法选取早孕需行人工流产的精神障碍患者且无明显器质性疾病者为入组对象,对照组为正常妊娠者,每组各62例。两组术前先肌注阿托品0.5mg,然后用微注泵静脉推注瑞芬太尼0.5μg/(kg min),丙泊酚1.5mg/kg。比较两组情况。结果①孕妇一般情况和术中麻醉情况两组比较差异无统计学意义(P>0.05);但观察组中丙泊酚的用量比对照组稍少。②孕妇诱导前后SpO2、HR及MAP的变化,两组比较差异无统计学意义(P>0.05),但两组诱导后1min内三者均有降低,与诱导前比较差异有显著性(P<0.05)。③孕妇不良反应情况两组比较差异无统计学意义(P>0.05),但呼吸抑制两组比较差异有统计学意义(P<0.05)。结论瑞芬太尼复合丙泊酚用于精神障碍孕妇无痛人流术中也能安全、快速、有效地终止妊娠,苏醒快,在临床上可推广应用。

Remifentanil Combined with Propofol Used for Psychiatric Observation of the Induced Abortion of Pregnant Wowen

Objective To observe remifentanil combined with propofol used in painless induced abortion in pregnant women with mental disorders safety and applicability. Methods Early pregnancy induced abortion for mental disorder patients without obvious organic disease into the set of objects, control group of normal pregnancy, 62 cases in each group. Two groups of intramuscular injection of atropine before 0.5mg, and then use the micro injection pump for intravenous infusion of remifentanil 0.5μg/（kg.min）, 1.5mg/kg propofol. Comparison of two groups. Results Pregnant women in general and intraoperative anesthesia no significant differences between the two groups （P〉0.05）; but in the observation group propofol dosage slightly less than the control group. The pregnant women before and after induction of SpO2, HR and MAP changes, no significant differences between the two groups （P〉0.05）, but the two groups after the induction oflmin three decreased, and before induction had significant difference （P〈0.05）I The adverse reactions of pregnant women no significant differences between the two groups （P〉0.05）, but the respiratory depression there was significant difference between two groups （P〈0.05） Conclusion Remifentanil combined with propofol used in painless artificial abortion of mental disorders in pregnant women can safely, quickly, effectively terminating the pregnancy, quicker recovery, in clinical on the popularization and application.
