芬太尼类麻醉性镇痛药在腹腔镜胆道手术中应用研究

摘    要:目的观察芬太尼类麻醉.I生镇痛药在腹腔镜胆道手术中应用效果。方法采用随机、开放的方法,选择90例腹腔镜胆道手术患者分为3组,每组30例。分别观察、记录镇痛、心血管系统和呼吸系统指标。结果①芬太尼类麻醉性镇痛药在此类手术中,瑞芬太尼(remifentanil)超短效、无蓄积特点,术后呼吸恢复快和无迟发呼吸抑制作用方面优于舒芬太尼(sufentanil)和芬太尼(fentanyl)。舒芬太尼镇痛持续时间长、呼吸抑制时间短、代谢快,麻醉质量和恢复质量优于芬太尼;②对心血管系统血压和心率影响,瑞芬太和舒芬太尼较芬太尼小。结论综合各方面因素,在腹腔镜胆道手术中应用瑞芬太尼会取得较舒芬太尼和芬太尼好的效果。

Abstract:Objective To observe the application effect ofthe Fentanyl category stupefactive analgesicsmedinces in the surgical operation of peritoneoscope biliary tract surgery. Methods to adopt a random and open method to choose 90 example surgery patients divided into 3 groups , each group of 30 examples, to observe and record analgesia, cardiovascular system and respiratory system indexs. Results (1)A Fentanyl kind of stupefactire analgesic in the surgery, Remifentanil has the characteristics of ultra-efficient and non-accumulation, it is superior than Sufentanil and Fentanyl in the respect of rapidly recovery respiration and non-delayed respiratory inhibition. Sufentanil analgesia lasts for a long time, respiratory depression time is short, metabolism is fast , its' the quality of restoration and anesthesia is better than fentanyl. (2)To the cardiovascular system blood pressure and the ehart rate influence, Remifentanil and Sufentanil is better than fentanyl. Conclusion Synthesizes various aspects factors, it will obtain better effect in the peritoneoscope biliary tract surgery using Remifentanil than Sufentanil and Fentanyl.
