低位腹主动脉球囊阻断技术在复杂骨盆和盆腔部位手术中的应用

目的 探讨低位腹主动脉球囊阻断技术在复杂骨盆和盆腔部位手术中的临床应用价值.方法 14例复杂骨盆和盆腔部位手术患者,术中采用低位腹主动脉球囊阻断技术,严密观察患者术中血流动力学变化,记录患者术中出血量,术前、术后凝血功能、肾功能的变化,并术后随访72 h.结果 手术过程顺利,血流动力学稳定.术中失血量50～4 000 ml,平均失血量928 ml,10例患者接受输血,输血量400～4 000 ml,平均输血量733 ml,4例患者未输血.手术前后凝血功能、肾功能指标差异无统计学意义.术后无肢体血栓形成、缺血性坏死及肾功能衰竭等并发症发生.结论 低位腹主动脉球囊阻断技术在复杂骨盆和盆腔部位手术操作简便,效果确切,有较好的临床使用价值.

Application of temporary balloon occlusion of lower abdominal aorta during complicated pelvic surgery

Objective To evaluate the clinical application of temporary balloon occlusion of lower abdominal aorta during complicated pelvic surgery.Methods Temporary balloon occlusion of lower abdominal aorta was used in 14 patients with complicated pelvic surgery.The hemodynamics,coagulation function,renal function were monitored and volume of bleeding during operation was recorded.All patients were followed up for 72 hours.Results The operations were all successful and the hemodynamics was stable.The mean volume of bleeding was 928ml,with a range from 50 to 4000ml.Ten patients received blood transfusion(400-4000ml),while 4 patients did not.There were no significant changes in coagulation and renal function preoperatively.No postoperative complication of embolism or ischemic necrosis was found in this group of patients.Conclusion Balloon occlusion of lower abdominal aorta technique is simple and effective during complicated pelvic surgery.
