电脑影像导航辅助下鼻内窥镜手术的麻醉特征分析

目的:研究电脑影像导航辅助下行鼻窦手术的最佳麻醉方法及控制性降压应用的可能性。方法:80例(ASAI—Ⅱ)鼻内窥镜下行鼻窦及副鼻窦手术患者,随机分成两组,观察组(n=40)静吸复合全麻,应用控制性降压技术,对照组(n=40)静吸复合全麻无控制性降压,对比两组术野的清晰度及术野出血情况.结果:两组患者在全麻下均顺利完成手术。观察组控制性降压后皮下组织出血较对照组明显减少(P<0.05),视野清晰,大大方便手术者操作。结论:采用静吸复合全身麻醉行控制性降压技术在影像导航手术中具有很好的效果,值得推荐。

Study on Anesthetic Characteristics of Computer - Aided Endoscopic Sinus Surgery

Objective: To investigate the optimal anesthesia computer - aided endoscopic sinus surgery and possibility of the use of controlled hypotension.Methods: 80 patients undergone endoscopic sinus and parasinus surgery were randomly divided into control group( n = 40) with combined general anesthesia of injection and inhalation, and observation group( n = 40) with general anesthesia of injection and inhalation combined controlled hypotension. Their operation field clarity and hemorrhage under subcutaneous tissue were measured. Results: All patients in two groups smoothly experienced the surgery performance, patients with the additive use of controlled hypotension had less hemorrhage ( P < 0.05) and clearer operation fields compared with those with no use of controlled hypotension . Thus, by the additive use of controlled hypotension, surgeons were more comfortable and convenient when they did their surgical performances. Conclusions:The effect of the anesthetic method of general anesthesia of injection and inhalation with controlled hypotension during the computer- aided endoscopic sinus surgery is good,and it deserves to be introduced to be widely used.
