腹腔镜胆囊大部切除术治疗急性化脓性胆囊炎

目的探讨急性化脓性胆囊炎行腹腔镜胆囊大部切除术的可行性及疗效。方法回顾性总结37例急性化脓性胆囊炎患者行腹腔镜胆囊大部切除术的临床资料。结果本组37例均成功行腹腔镜胆囊大部切除术,无中转开腹。术后因肝肾功能衰竭死亡2例,腹膜炎中毒性休克死亡1例;发生胆汁漏4例,均在1周之内经保守治疗痊愈。结论特殊情况下行腹腔镜胆囊大部切除术治疗急性化脓性胆囊疾病是安全可行的,有一定的临床价值。

Laparoscopic Subtotal Cholecystectomy for the Acute Suppurative Cholecystitis

Objective To explore the clinical value of the laparoscopic subtotal cholecystectomy for acute suppurative cholecystitis.Method The clinical data of 37 cases of acute suppurative cholecystitis underwent laparoscopic subtotal cholecystectomy was retrospectively analyzed.Results All the operations were completed successfully without conversion to open surgery.2 cases died of kidney and liver failure,and another one died of peritonitis followed by toxic shock.4 cases suffered from bile leakage after operation and were cured by conservative management within one week.Conclusion In some special acute suppurative cholecystitis cases,the laparoscopic subtotal cholecystectomy is safe and feasible.
