妊娠期糖尿病的处理(附56例报告)

目的探讨妊娠期糖尿病(gestational diabetes medlitus,GDM)的处理方法。方法将56例 GDM(观察组)采用饮食和胰岛素治疗与60例正常孕妇(对照组)比较。结果观察组有35例采用饮食疗法后血糖控制良好,21例饮食疗法加胰岛素治疗血糖控制在正常范围内。观察组终止妊娠的周数比对照组短(P<0．05),剖腹产率、新生儿体重高于正常对照组(P<0．05),妊娠期高血压、感染、产后出血、胎儿生长受限、新生儿窒息、新生儿高胆红素血症等发生率分别与对照组比较无差异(P>0．05)。结论加强GDM患者孕期监护、饮食控制,并在必要时加用胰岛素治疗,可以有效降低母婴并发症,改善围产儿的预后。

Treatment for 56 Cases of Pregnant Women with Gestational Diabetes Mellitus

Objective To discuss the treatment for pregnant women with gestational diabetes mellitus. Methods 56 cases of gestational diabetes mellitus compared with 60 cases of normal pregnancy were conducted. Results Among 56 cases of gestational diabetes mellitus, there were 35 cases whose blood sugar to be controlled within normal limits only by dietary therapy ,and the other 21 cases' blood sugar to be controlled within normal limits by insulin treatment combined with dietary therapy. Through clinical treatment, 56 cases of gestational diabetes mellitus whose newborn infants' body weight and premature labor were higher than that of normal control group (P <0.05).The incidences of hypertensive disorder complicating pregnancy, infection, postpartum hemorrhage, polyhydrimnios, oligohydramnios, neonatal asphyxia hyperbilirubinemia of newborn and so on in the 56 cases gestational diabetes mellitus were no difference compared with normal pregnant women group (P >0.05). Conclusion The pregnant women with gestational diabetes mellitus should strengthen monitoring duration of pregnancy and alimentary control only or alimentary control added insulin treatment could cut down complications of mothers and fetals.
