椎弓根螺钉内固定植骨治疗胸腰椎骨折

目的总结胸腰椎骨折的治疗经验，减少术后并发症的发生。方法选36例胸腰椎骨折病例：男27例女9例；年龄20～66岁。损伤节段；T1211例，L112例，L28例，L32例L43例。伤后至手术时间1～16天，均为爆裂骨折。术前椎管占位率35％-80％。脊柱后凸程度（Cobb’s角）：20～40度。椎体高度压缩至正常的30％～70％。Frankel分级：A级2例，B级4例，C级6例，D级8例，E级16例。治疗按置钉、减压、复位、植骨步骤进行。结果随访6～18个月，椎体高度恢复至正常的90％～100％。椎管占位率2％-10％。Cobb角平均3度，Frankel分级：A级1例，B级2例，C级3例，D级7例，E级23例。结论椎弓根螺钉内固定并通过椎弓根向椎体内植骨治疗胸腰椎骨折，可早期重建脊柱稳定，改善脊髓神经功能，为前柱提供长期稳定，避免复位后形成“蛋壳”椎体，减少胸腰椎骨折术后并发症的发生。
