心脏瓣膜替换术156例

心脏瓣膜替换手术１５６例，严格作好术前准备，改善心肺功能，对重症患者手术时机选择，术中加强心肌保护，妥善处理继发病变，术后重视心肺功能支持治疗。对左室舒张末期内径大于６．０ｃｍ和尽量保留二尖瓣瓣下结构并在术后预防性应用利多卡因。

AN ANALYSIS OF 156 CASES OF CARDIAC VALVE REPLACEMENT

156 patients underwent cardiac valve replacement between 1990 and 1995. The careful preparation of preoperation were taken to improve cardiac function. The operation time was rigorously set for the seriously ill patients. The results showed that 7 patients died in the early stage, 1 patient died in the later stage. The inhospital mortality was 4. 5%. The satisfying outcome was obtained. Authors emphasized the importance of myocardial protection during operation and cardiopulmonary function care after the operation. The subvalular structures were retained as fully as possible for the patients with left ventricular internal dimension at end diastole with over 600mm.Lidocaine was routinely administrated prophylactically.
