原发性血小板增多症31例临床分析

目的探讨原发性血小板增多症(ET)的临床表现、诊断、治疗及预后。方法对本院2005年3月至2009年3月间诊治的31例ET患者资料进行回顾性分析。结果首诊入住血液内科者仅占54.8%,首诊入住其他科室者达45.2%;临床表现仅有头晕、乏力等非特异性症状者占80.6%,因其他原因做血常规确诊本病者占19.4%。38.7%患者合并栓塞,但合并出血者占3.2%。JAK2/V617F基因检测突变者52.6%。羟基脲治疗有效率(CR+PR)达81.0%,羟基脲+干扰素治疗有效率达90.0%。常规治疗组初治未合并血栓者血栓发生率77.8%,奥扎格雷钠治疗组初治未合并血栓者血栓发生率20.0%。结论ET患者因临床表现缺少特异性容易漏诊,其栓塞发生率大于出血发生率。多数ET患者存在JAK2/V617F基因检测突变。羟基脲+干扰素的治疗可获得较好疗效。血栓素A2抑制剂奥扎格雷钠干预治疗可以减少ET患者血栓发生率。

Clinical Analysis of 31 Patients with Essential Thrombocytosis
