摘    要:目的:探讨经尿道等离子体双极电切术同期治疗膀胱肿瘤合并良性前列腺增生的可行性。方法:23例良性前列腺增生合并膀胱肿瘤患者,行同期经尿道前列腺及膀胱肿瘤等离子体双极电切术并随访观察疗效。结果:术后均未发生经尿道电切综合征,术后3个月患者国际前列腺症状评分由术前的23.6±7.2下降至5.1±0.9,最大尿流率由术前的(5.6±3.4)mL/s提高至(20.2±3.8)mL/s,差异有显著性(P<0.05)。随访5~52个月,3例术后膀胱肿瘤复发,复发部位均不在后尿道、前列腺窝。结论:膀胱肿瘤合并前列腺增生患者采用经尿道等离子体双极电切术同期手术治疗安全、可靠,免除了患者二次手术的痛苦。

Abstract:Objective To evaluated the effect of simultaneous transurethral resection of benign prostatic hyperplasia (BPH) and bladder transitional cell carcinoma (BTCC) with bipolar plasmakinetic technique. Methods 23 patients with BTCC and BPH had undergone plasmakinetic resection of the bladder tumor (PKRBT) and plasmakinetic resection of the prostate (PKRP) at the same time, and had been followed up postoperatively to view the result. Results No transurethral resection syndrome occurred in those patients. The peak flow rate increased from (5.6 ± 3.4) mL/s to (20.2 ± 3.8) mL/s, the international prostate symptom score (IPSS) decreased from 23.6 ± 7.2 to 5.1 ± 0.9 at postoperative 3 months respectively and made difference (P < 0.05). The following up period ranged from 5 ~ 52 months. The recurrence occurred to 3 cases without implancation at the bladder neck and prostatic urethra metastase. Conclusions PKRBT and PKRP at the same time is a safe,effective,reliable method for patients with BTCC and BPH. It can release painness of patients for second operation.
