视觉诱发电位联合智力测验对亚临床肝性脑病的诊断价值

目的 :探讨视觉诱发电位联合智力测验对亚临床肝性脑病的诊断价值。方法 :对 6 5例肝硬化患者 (ChildA级 18例 ,ChildB级 2 7例 ,ChildC级 2 0例 )进行脑电图 (EEG)、视觉诱发电位 (VEP)测定及智力测验 (PT) ,并与 47例正常人进行对照 ,研究其诊断价值。结果 :①肝硬化患者EEG异常者 2 1例 (32 %) ,对照组异常者 10例 (12 %) ,两者对照显示无显著差异 (P >0 0 5 )。②肝硬化患者单独VEP或PT异常者分别为 18例 (2 8%)和 9例 (14%) ,两者共同异常者 16例 (2 5 %) ,两者总阳性率为77%。对照组单独VEP、PT异常者分别为 12例和 7例 (2 6 %、15 %)。两者对照显示高度显著差异 (P <0 0 1)。结论 :视觉诱发电位联合智力测验对亚临床肝性脑病的检出有较高的敏感性。

The diagnostic value of subclinical hepatic encephalopathy in patients with cirrhosis determined by VEP and Psychometric test

Objective To investigate the diagnostic value of subclinical hepatic encephalopathy(SHE) in patients with cirrhosis determined by VEP and Psychometric test.Method EEG and VEP,PT were carried out in sixty-five cirrhotic patients (child-pugh A:Eighteen palients,child-pugh B:twenty-seven,child-pugh C:twenty Patients),and fourty-seven normal person were controlled at the same time,study their clinical value.Results (1)Twenty-one patients were abnormal in EEG and ten of control group were abnormal in EEG.The difference was insigniffcent between the patients groups and control group(P>0.05).(2)Eighteen(28%) patients were abnormal only in VEP and Nine(14%) patients were abnormal only in PT,Sixteen(25%) patients were abnormal in both VEP and PT,total positive rate(77%).Twelve(26%) and Seven(15%) were abnormal individully in VEP and PT.The difference was significent in VEP and PT between the patients group and control group(P<0.01).Conclusion VEP and PT were higher sensitivity in diagnosis of SHE.
