老年肾移植患者术前严格诊断性筛查的意义

目的根据老年尿毒症患者的特点,通过手术前各项指标的严格诊断性筛查,提高肾移植手术的成功率及移植后人/肾的长期存活率。方法常规诊断性筛查组：2007年10月至2008年10月间,对在我院行肾移术的20例60岁以上老年尿毒症患者,术前进行常规诊断性筛查;严格诊断性筛查组：2008年10月至2009年10间,对在我院行肾移植术的23例60岁以上老年尿毒症患者,术前进行严格诊断性筛查。通过各项检查结果诊断患者有无肾移植禁忌症;对于通过常规诊断性筛查,但未达到严格诊断性筛查标准的老年尿毒症患者,给予充分透析,增加营养,控制感染,调整心、肺功能,积极治疗原发病等治疗,达到严格筛查标准后,方可行肾移植术。结果 2007年10月至2008年10月间,通过常规筛查而行肾移植的20例老年患者中,有2例分别于术后3个月、5个月死于肺炎,1例与术后2个月死于心脏病;2008年10月至2009年10月间,经过严格筛查而行肾移植的23例老年患者中,除1例于术后4个月死于CMV肺炎,其余患者全部存活。结论随着医学诊断水平的提高以及新型免疫抑制剂的应用,高龄尿毒症患者同样适合行肾移植手术,但受者多脏器的功能状态是影响移植效果的主要因素之一。在术前对受体进行严格的诊断性筛查,是老年肾移植成功的关键。
