血清肿瘤标志CEA和CA724联合检测在结直肠癌诊断中的应用

目的探讨癌胚抗原(CEA)和糖链抗原724(CA724)联合检测对结直肠癌的诊断价值。方法用化学发光法检测结直肠癌患者41例、结直肠良性病变患者46例、健康体检人群101例的血清CEA含量,用电化学发光法检测3组人群的血清CA724水平,统计分析CEA和CA724在结直肠癌诊断中的应用价值。结果结直肠癌患者血清CEA和CA724水平显著高于结直肠良性病变患者及健康体检人群的水平(P<0·05);结直肠良性病变患者血清CEA和CA72-4浓度显著高于健康体检人群的浓度(P<0·05)。单项检测CEA对结直肠癌诊断的敏感度、特异性和准确性分别为58·5%、76·0%、67·8%;CA724为:34·1%、93·4%、65·5%;联合检测CEA和CA724为:68·2%、71·7%、70·1%。结论 CEA和CA724结直肠癌诊断和疗效监测有一定价值,两者联合检测可提高结直肠癌的检出率。

Application of combined determination of CEA and CA724 to the diagnosis of colorectal cancer

Objective To investigate the value of combined determination of CEA and CA724 in the diagnosis of colorectal cancer Methods Serum CEA in 41 patients with colorectal cancer,46 patients with benign disease of colorectal and 101 healthy adults were measured with chemiluminescence assay Serum CA724 of the same groups were detected by electrochemiluminescence assay The value of CEA and CA724 in the diagnosis of colorectal cancer was analyzed Results Serum CEA and CA724 in patients with colorectal cancer were significantly higher than those in patients with benign disease of colorectal and the healthy（P0.05） Serum CEA and CA724 in patients with benign disease of colorectal were significantly higher than those in the healthy（P0.05）.The sensitivity,specificity and accuracy of CEA in the diagnosis of colorectal cancer were：58.5%,76.0% and 67.8% Those of CA724 were 34.1%,93.4% and 65.5% Combined determinatin of CEA and CA724 were 68.2%,71.7% and 70.1% Conclusion CEA and CA724 are useful tumor marker for diagnosis and monitoring of therapy effect of colorectal cancer Combined determination of CEA and CA724 is helpful to increase detection rate of colorectal cancer.
