滑膜软骨瘤病的影像学表现及其病理基础

目的：探讨滑膜软骨瘤病（SC）影像学表现及其病理基础。方法：回顾性分析经关节镜及病理证实的16例SC患者的影像资料,并与病理结果对照。结果：16例SC中累及膝关节12例（双膝关节同时累及1例）,累及踝关节2例,累及髋关节2例。16例均可见滑膜肥厚、关节内游离体及关节积液;增厚滑膜T1WI、FFE呈等或稍低信号,T2WI呈稍高信号;未钙化游离体T1WI、T2WI均呈中等信号,钙化游离体T1WI、T2WI均呈低信号,骨化游离体T1WI呈高信号、T2WI呈中等信号。部分病例见骨髓水肿、交叉韧带、半月板损伤及关节周围软组织肿胀。结论：MRI能够提供比较全面的影像学信息,可提高SC的诊断符合率,指导临床治疗。

MR imaging manifestations of synovial chondromatosis and pathologic basis

Objective：To evaluate the MR imaging manifestations and their pathologic basis of synovial chondromatosis （SC）.Methods：The MR imaging materials of 16 cases with SC proved by arthroscopy and pathology were collected and retrospectively analyzed as well as correlated with pathology.Results：Of the 16 cases,there were involvement of knee joint （n=12;simultaneous bilateral joint involvement,n=1）,ankle joint （n=2）,hip joint （n=2）.Synovial membrane hypertrophy,intra-articular loose bodies and joint effusion could be assessed in all of the 16 patients.Thickening of the synovial membrane showed iso-/slight hypo-intensity on T1WI and FFE,slight hyper-intensity on T2WI;non-calcified loose bodies showed moderate intensities on T1WI and T2WI;calcified loose bodies showed hypo-intensities on T1WI and T2WI,and ossified loose bodies showed hyper-intensity on T1WI,moderate intensity on T2WI.Bone marrow edema,posterior cruciate ligament and meniscus injury,peri-articular soft tissue swelling could be revealed in part of the patients.Conclusion：Comprehensive information of SC could be obtained on MRI,which can improve the accuracy of diagnosis and provide guidance of clinical treatment.
