氯沙坦联合硝苯地平对老年2型糖尿病肾病合并高血压患者肾功能的影响

目的 观察氯沙坦联合硝苯地平对老年2型糖尿病肾病合并高血压的疗效及对肾功能的影响.方法 将2009年12月至2010年12月收治的86例2型糖尿病肾病合并高血压患者随机均分为两组,观察组43例服氟沙坦片联合硝苯地平控释片治疗,对照组43例仅口服硝苯地平控释片治疗,观察两组的降压效果及对肾功能的影响.结果 观察组的降压总有效率为72.09%,明显高于对照组的51.16%(P<0.05).与治疗前比较,观察组治疗后血尿素氮、血肌酐、血尿酸和尿微量白蛋白水平明显下降(P<0.05),且均明显低于对照组(P<0.05).结论 氯沙坦联合硝苯地平治疗老年2型糖尿病肾病合并高血压,降压效果明显,且可保护肾脏功能,值得临床推广.

Effect of Losartan Combined with Nifedipine in Treating Type 2 Diabetic Nephropathy Complicating Hypertension and Its Affection on Kidney Function

Objective To observe the effect of losartan combined with nifedipine in the treatment of type 2 diabetic nephropathy（DN） complicating hypertension and its affection on kidney function. Methods From December 2009 to December 2010, 86 cases of type 2 DN complicating hypertension were randomly divided into two groups. The observation group（43 cases） was treated with losartan com- bined with nifedipine and the control group （43 cases） was treated with nifedipine only. The antihypertensive effects and the kidney function before and after treatment were observed. Results The total effective rate of antihypertension in the observation group was significantly higher than that in the control group（P 〈 0.05）. Compared with pre-treatment, the levels of serum urea nitrogen, creati- nine, uric acid and urine microdose albumin were significantly decreased in observation group（ P 〈 0.05）, which had no significant difference before and after treatment in the control group（ P〉 0.05）. After treatment, the levels of serum urea nitrogen, creatinine, uric acid and urine microdose albumin in the observation group were significantly lower than those in the control group（P〈 0.05）. Conclusion Losartan combined with nifedipine is significantly effective in the treatment of type 2 DN complicating hypertension and can protect the kidney function, which deserves to be popularized in clinic.
