棘间入路法作腰椎间盘摘除术

目的：探讨经棘突间入路法作腰椎间盘摘除的应用价值。方法：作者１９９６ 年１１ 月～１ ９９８ 年５ 月作了１５ 例，男性８ 例，女性７ 例，平均３８ 岁，均有下腰腿痛史， Ｃ Ｔ 明确诊断。采用此入路作腰椎间盘摘除，６ 例同时作侧隐窝扩大，神经根管松解，１ 例作了二个间盘摘除。结果：平均随访１ 年， 优９ 例，良４ 例，中、差各１ 例。疗效满意。棘间入路优点为经棘间、椎间隙即可进入腰椎间盘与黄韧带间隙，简称腰盘黄间隙，易于摘除腰椎间盘。结论：此入路简易、创伤小，有利于保持脊柱稳定。

Removal of lumbar disc herniation through the interspinal approach

Aim:To study the applied value of removing lumbar disc herniation through the interspinal approach.Methods:15 patients with lumbar disc herniation were treated by this method from December 1996 to May 1998. Thy were 8 males and 7 females and the average age was 38 years.All patients had low back pains and their diagnosis were confirmed by CT.The lateral recesses were expanded and nerve roots were released simultaneously in 6 of them; 2 herniations were removed at the sametime in 1 case. Results:One year follow up showed that 9 cases were excellent.4 good,1 fair and 1 poor.The results were satisfactory.Conclusions:The features of this approach are ease access of the space between discus intervertebralis and flavum (named I D F apace).So its outstanding advantage,include simplicity,effectiveness and preserving stability of the vertebral column.
