流程管理在急性心包填塞急救护理中的应用

目的 探讨流程管理在急性心包填塞急救护理中应用的效果。 方法 将2015年1月-2016年6月收治的10例患者作为对照组,采用常规急救护理;将2016年7月-2017年12月收治的11例患者作为观察组,采用流程管理,具体方法包括使用“心包穿刺集束箱”提前准备急救用物,优化人员配备及救护流程,保障抢救有序进行,落实培训及考核,提高护理人员急救技能,应用“抢救护理记录单”帮助管床护士事后高效补记抢救记录。 结果 2组在用物准备时间、一次备物的缺件数量、参与抢救护理人员数量方面比较,差异均有统计学意义(Z=-3.844,P<0.001; Z=-3.889,P<0.001; Z=-3.924,P＜0.001),培训后流程管理组理论成绩、情景模拟演练术前准备完成时间、补记抢救记录花费时间均优于对照组(t=2.380,P=0.024;t=11.460,P<0.001; t=2.520,P=0.021)。 结论 流程管理能缩短心包穿刺用物准备的时间、提高一次备物成功率,避免人力资源浪费、提高低年资护士应急抢救能力、缩短抢救记录完成的时间。

The application of process management in the emergency nursing of patients with acute pericardial tamponade

Objective To evaluate the efficacy of applying the process management in the emergency nursing of patients with acute pericardial tamponade (PT). Methods Ten PT patients treated between January 2015 and June 2016 were selected into a control group, while 11 treated between July 2016 and December 2017 were chosen into an observation group. The control group was given routine emergency nursing, while the observation group was provided with emergency nursing based on the process management, including using the pericardicentesis bundle box, optimizing the personnel allocation and rescuing process, implementing training and evaluation, as well as modifing the emergency nursing list. Results Significant differences were observed between the two groups with respect to the mean set-up time, numbers of rescue equipment not prepared and numbers of nurses taking part in the rescuing (Z=-3.844,P<0.001; Z=-3.889,P<0.001; Z=-3.924,P＜0.001). Moreover, the theoretical grade, and time taken for preparation in simulation exercise and recording the rescuing process of the experimental group were significantly superior to the control group (t=2.380,P=0.024;t=11.460,P<0.001; t=2.520,P=0.021) . Conclusion The process management of nursing care for PT patients can shorten the preparation time for conducting pericardicentesis, promote the success rate of one-time preparation, avoid the wasting of nursing staff, promote the emergency treatment of junior nurses and shorten the time for filling in the record.
