加速康复外科理念在复杂肛瘘患者围手术期护理的应用效果

目的 探讨加速康复外科理念在复杂肛瘘患者围手术期护理的应用效果。 方法 选取2016年1月-2017年7月在我院进行治疗的复杂肛瘘患者134例,采用随机数字表法将患者分为观察组(67例)、对照组(67例)。对照组采用常规护理,观察组采用加速康复外科护理。采用视觉模拟评分法(visual analogue scale/score,VAS)比较2组疼痛程度、创面愈合时间、并发症(尿潴留、排便不畅、肛门坠胀)发生率、护理满意度。 结果 观察组VAS评分明显低于对照组(F_时间=1 342.02,P<0.001;F_组间=55.25,P<0.001;F_交互=9.36,P<0.001),创面愈合时间短于对照组(t=8.09,P=0.02),尿潴留、排便不畅、肛门坠胀发生率均低于对照组,观察组护理满意度高于对照组(Z=-3.449,P=0.001)。 结论 复杂肛瘘住院患者接受加速康复外科护理可有效改善术后疼痛,缩短住院时间和创口愈合时间,减少尿潴留、排便不畅、肛门坠胀等并发症的发生,提高患者的护理满意度。

Effects of applying accelerated rehabilitation surgery in perioperative nursing for patients with complex anal fistula

Objective To explore the effect of applying the accelerated rehabilitation surgery in the perioperative nursing of patients with complicated anal fistula. Methods A total of 134 patients with complicated anal fistula treated in our hospital between January 2016 and July 2017 were randomly divided into an observation group (n=67) and a control group (n=67) according to a random number table. The control group received routine nursing, while the observation group was provided with nursing care according to the concept of the accelerated rehabilitation surgery. Both groups were recorded the visual analogue scale (VAS) scores, the time of wound healing, the duration of hospitalization, the incidence of complications such as the urinary retention, defecation and anus distention), and nursing satisfaction. Results The VAS score of the observation group was significantly lower than that of the control group. The wound healing time of the observation group was significantly shorter than that of the control group (t=8.09, P=0.02). The incidence of urinary retention, poor defecation and anal bulge was significantly lower than that of the control group. The satisfaction rate of the former group was significantly higher than the latter (Z=-3.449, P=0.001). Conclusion Patients with complex anal fistula undergoing nursing according to the accelerated rehabilitation surgery can effectively relieve the postoperative pain, shorten hospitalization and wound healing time, reduce such complications as the urinary retention, poor bowel movements and anal bulge, and improve their satisfaction.
