宫颈冷刀锥切术诊治宫颈病变的临床分析

目的分析宫颈锥切术诊治宫颈上皮内瘤变(C IN)的效果及其价值。方法回顾性分析2006年8月至2010年10月49例C IN患者的临床资料。选取C INⅡ级、Ⅲ级的36例患者做阴道镜下多点活检及其宫颈锥切活检或宫颈锥切术,运用自身对照法行病理组织诊断分析。结果 36例患者中宫颈多点活检与宫颈锥切病理结果报告符合率61.11%(22/36)。宫颈锥切术术后出血3例,经电凝止血或局部压迫止血治愈。随访3~12个月均无细胞学检查异常。结论宫颈锥切术能有效弥补阴道镜下活检诊断宫颈病变的缺陷,大大提高诊断的准确率;治疗效果佳,不失为一种有效的诊治方法,值得临床应用。

Study on cervical cold knife conization in diagnosis and treatment of cervical lesions

Objective To analyze the effect and significance of cervical conization in diagnosis and treatment of cervical intraepithelial neoplasia（CIN）.Methods The data of 49 patients with CIN treated in this department during August 2006 to October 2010 were retrospectively analyzed.Thirty six patients with grades CIN Ⅱ and Ⅲ were taken with colposcopy and cervical biopsy under multi-cone biopsy or cervical conization,and self-control method was applied for pathological tissue diagnosis.Results The coincidence rate of thirty six patients with cervical multi-cone biopsy and pathological report reached 61.11%（22/36）.Postoperative bleeding after conization was seen in 3 cases,and they were cured by coagulation hemostasis or local pressing.They had been followed up for 3 to 12 months,no cytological abnormalities had been found.Conclusion Conization can effectively remedy the defects of colposcopic biopsy in diagnosis,and it may greatly improve the accuracy of diagnosis and raise the effectiveness of treatment,it is an effective diagnostic and therapeutic method,hence it is worthy to be recommended for clinical application.
