摘    要:目的观察总前列腺特异抗原(TPSA)、游离前列腺特异抗原(FPSA)、FPSA与TPSA比值(F/T)在良性前列腺增生(BPH)病理类型诊断中的作用。方法检测98例BPH(腺瘤样型39例,纤维肌型35例,纤维肌腺瘤样型24例)、30例健康患者(对照组)的TPSA、FPSA,并计算出相应的F/T值。结果 BPH组FPSA、TPSA均显著高于对照组,F/T低于对照组,差异具有显著性(P<0.05)。BPH各组分型相比较发现,腺瘤样型组患者血清FPSA、TPSA高于纤维肌型组、纤维肌腺瘤样型组,差异均具有显著性(P<0.05)。其中纤维肌型组与纤维肌腺瘤样型组比较,患者血清FPSA、TPSA均无明显差异。F/T在各BPH分型中差异均无显著性(P>0.05)。结论腺瘤样型BPH患者血清FPSA、TPSA高于纤维肌型和纤维肌腺瘤样型,对诊断BPH病理类型有一定程度上诊断价值,从而为临床指导用药提供参考。

Abstract:Objective To observe the serum total prostate specific antigen(TPSA),free prostate specific antigen(FPSA),and the ratio of FPSA to TPSA ratio(F/T) in the diagnosis of pathological types of benign prostatic hyperplasia(BPH).Methods Ninety-eight cases of BPH(adenomatoid type 39 cases,muscle fiber type 35 cases,fibromuscular adenomatoid type 24 cases) and 30 healthy subjects(control group) were included in this study.Serum TPSA and FPSA were detected and the corresponding F/T value was calculated.Results FPSA and TPSA were significantly higher,F/T was lower in BPH group than the control group(P0.05).Comparison of serum levels of FPSA and TPSA in different pathological types of BPH showed that serum FPSA and TPSA were higher in adenomatoid type than those in muscle fiber type group and fibromuscular adenomatoid type group(P0.05),however,there were no differences in the levels of FPSA and TPSA between muscle fiber group and fibromuscular adenomatoid group(P0.05).In addition,there was no significant difference in F/T among each type of BPH(P0.05).Conclusion The finding that serum FPSA and TPSA levels were higher in patients with adenomatoid type BPH than those in patients with muscle fiber type and fibromuscular adenomatoid type BPH may provide help for the diagnosis of pathological types of BPH and for the treatment in clinics.
