牙髓腔-骨内种植体断裂的临床和超微结构分析

摘    要:目的 :通过对牙周病松动牙髓腔 -骨内种植术固定 15年后种植体断裂的原因分析 ,为临床应用和种植体材料的改进提供依据。方法 :检查患牙、牙周状况及咬合关系 ,取出牙槽骨内和髓腔内的种植体 ,扫描电镜超微结构观察和材料表面元素能谱分析 ,综合分析种植体断裂的原因。结果 :患牙有明显的咬合创伤、深牙周袋 ,种植体两断端均呈现疲劳纹 ,元素能谱分析证实骨内段表面Ca、P含量比例增高。结论 :种植体与骨组织有良好的生物相容性和骨整合性 ,种植体断裂与长期应用后的材料疲劳相关 ,患者术后没有坚持进行牙周维护治疗可能是主要影响因素。

Abstract:Objective;To observe the micro-structure of a fractured pulp-osseous implant for treating mobilized periodontitis tooth and analyses the ponible causes. Method:Both the implants in bone and in pulp were extracted and washed by distilled water. The micro-structure of the cross sections of both parts were observed by SEM and analyzed by EDAX. Result: Fatigued source and typical developed stripe were showed on the fractured implant . The implant in bone showed higher percentage of Ca,P in the surface attachment. Conclusion: The implant has excellent biocompatibility and osseointegration. The fracture of implant in this case was partly caused by metal fatigue and most probebly caused by lack of periodontal maintenance.
