广东省乙型肝炎病毒S基因变异与乙型肝炎疫苗免疫失败关系探讨

摘    要:目的研究广东省儿童乙型肝炎(乙肝)病毒(HBV)表面抗原(HBsAg)携带者中HBV S基因变异对乙肝疫苗免疫效果的影响。方法收集乙肝疫苗免疫后HBsAg阳性的儿童血清标本36份,利用聚合酶链反应(PCR)扩增HBV S基因片段,对PCR产物进行脱氧核糖核酸(DNA)序列测定。结果在36份标本中,HBV核酸阳性26份,完成测序23份,11份标本S基因片段出现氨基酸变异,其中4份在“α”决定簇片段内出现了氨基酸变异,2种变异形式未见报道,结论广东省免疫失败儿童存在不同位点的HBV S基因变异,可能是导致部分儿童免疫失败的原因。尚未出现稳定的变异株,对现用乙肝疫苗的免疫效果影响不大。

Abstract:Objective To investigate the effect of hepatitis B virus S gene mutation to breakthrough infections children immunized with hepatitis B vaccine in Guangdong Province.Methods HBV DNA S gene were amplified by polymerase chain reaction(PCR) in sera from 36 children with HBsAg positive after immunized with hepatitis B vaccine.The PCR products were sequenced.Results Twerty six cases in 36 were PCR positive,23 of which were sequenced.The amino acids mutation were presented in 11 samples.Four samples presented amino acid mutation in "a"determinant of HBV DNA S gene.Two mutants have not been reported.Conclution Different variants cause HBV Vaccination failure in some children of Guangdong Province.No constant variant was found.Current vaccination strategies can still be carried out.
