胃癌变过程中幽门螺旋杆菌感染与COX-2、P53蛋白表达关系及其临床意义

目的：通过检测幽门螺旋杆菌（Helicobacter Pylofi，HP）感染胃粘膜组织环氧合酶-2（Cyclooxygenase-2，COX-2）和抑癌基因P53表达，探讨幽门螺旋杆菌相关胃癌的发生机制。方法：用快速尿素酶试验及改良Giemsa染色方法检测HP感染，COX-2表达检测选择免疫组化S-P法，用免疫组化LSAB法检测P53蛋白。结果：HP阳性感染率、COX-2和P53的阳性表达率，慢性萎缩性胃炎、肠化、非典型增生及胃癌中均明显高于慢性浅表性胃炎（P〈0．01）。慢性萎缩性胃炎、肠化、非典型增生及胃癌中HP感染与COX-2、P53的表达呈正相关（P〈0．01）。结论：COX-2、P53是HP感染胃癌变过程的早期事件，并参与了胃癌变的全过程。COX-2、P53蛋白可用为筛选胃癌高危癌前病变的标志物。

Study on Helicobcater Pylori infection , expression of COX-2 and P53 protein on the pathogenesis of gastric cancer

Objective:To explore the mechanism of Helicobacter Pylori(HP)-related gastric carcinogenesis via examining Cyclooxygenase-2(COX-2) and P53 expression in the affected gastric mucosa.Methods:HP was detected by using rapid urease test and modified Gemsa stain,COX-2 expression were detected by immunohistochemistry,LSAB immunohistochemical study on P53 protein was carried out by using monoclonal antibody(DO-1) to P53.Results:HP infection rate,COX-2 positive expression rate and P53 positive expression rate in chronic atrophic gastritis,intestinal metaplasia,dysplasia and gastric carcinoma specimens were significantly higher than those in chronic superficial gastritis(P<0.01).Expression of COX-2 and P53 were positively correlated with HP infection in chronic atrophic gastritis,intestinal metaplasia,dysplasia and gastric carcinoma(P<0.01).Conclusions:COX-2 and P53 were the early event during the course of gastric cancerization after Hp infection;COX-2 and P53 took part in the total process.COX-2 or P53 protein may be a marker of high risk precancerousof gastric lesions.
