摘    要:目的 探讨全脑室系统出血铸型的理想治疗方法.方法 采用双侧侧脑室持续体外引流及脑室内尿激酶灌注并配合腰大池置管脑脊液净化治疗108例全脑室系统出血铸型患者,治疗期间严格控制感染.结果 90例存活,18例死亡.90例复查CT显示脑室积血于4~12 d消失.6个月后随访:恢复良好54例,轻瘫18例,重瘫18例,CT示脑室系统正常.结论 该治疗方法操作简单、微侵袭并且明显降低死亡率,提高了生存质量,尤其适合全脑室出血铸型患者,值得推广应用.

Abstract:Objective To investigate the method of treatment of total intraventricular hemorrhage (TIH) of casting from. Methods 108 patients were treated with continuous bintraventricular external drainage, intraventricular administration of urokinase and purification of their CSF by lumbar puncture and catheterization. Infection was control strictly within the period of drainage. Results In this group,90 patients survived and 18 patients died o Head CT were reexamined about the 90 cases, which showed disappearance of intraventricular hematoma within 4 - 12days o Follow - up studies were made 6months after the treatment :54 cases lived in good conditions, 18 were mildly paralysis and the other 18 were severely paralysis, normal ventricular system on CT. Conclusion This method of the treatment may serve as an easy and little wound and effective way to deliver a saving measure to the patients with total intraventricular hemorrhage of casting from, and the mortality rate can be reduced greatly and the quality of survival can he improved. It's worth widely using.
