不同性别急性心肌梗死患者3h内静脉溶栓的临床疗效分析

目的探讨不同性别急性心肌梗死（AMI）患者3h内静脉溶栓近期疗效的差异及危险因素。方法选择本院心内科收治的符合静脉溶栓适臆证，无禁忌证的AMI患者共98例。根据性别分为男性组76例，女性组22例。两组均于3h内给予静脉溶栓治疗。结果（1）接受静脉溶栓治疗的急性心肌梗死（AMI）患者男性年龄较女性提前（P〈0．05）；（2）两组住院病死率无明显差异（P〉0．05）；（3）男性梗死相关血管再通率大于女性（P〉0．05）；（4）AMI易患因素中，高血压因素两组比较无显著性差异（P〉0．05）；女性高血脂及糖尿病高于男性（P〈0．05）；男性吸烟史显著高于女性（P〈0．01）；（5）女性组并发心力衰竭及房室传导阻滞明显高于男性（P〈0．05）。结论急性心肌梗死（AMI）患者3h内静脉溶栓近期疗效，女性不如男性。

The analysis of clinical effects about sex diference in acute myocardial infarction used tyrombolytic therapy introvenously within three hours

Objective The aim of this study was the clinical effect of intravenous thrombolysis therapy on sex difefence in acute myocardial infarction （AMI） within three hours. Methods In Department of Cardiovascular Medicine of our hospital,98 patients with AMI were divided into two groups of men and women, All patients were treated with intravenous thrombolysis therap within three hours. Results （ 1 ） The age of women was larger than that of men （ P 〈 0.05 ） ;（2） Hospital mortality in two groups was not significantly different （ P 〈 0.05 ） ; （ 3 ） Men infarct - related artery recanalization rate than females. （4） In the history of diseases, the history of hypertension in two group was not significantly different （ P 〉 0. 05 ）, the history of diabetes and hyperlipldemia for female was higher than those for male （ P 〈 0.05 ）, and the history of smoking for female was lower than male （ P 〈 0.05 ） ; （ 5 ） Complications （ left ventricular failure and auriculo - ventricular block） of AMI for female were higher than male （ P 〈 0. 05 ）. Conclusion AMI patients treated with intravenous thrombolysis therapy get a ben- eficial effect within three hours is more obvious in men than in women.
