宫腔内人工授精266周期疗效分析

目的　总结 2 6 6周期宫腔内人工授精 (IUI)的临床效果。方法　回顾分析我院 1999年 6月— 2 0 0 2年 4月 2 0 3对不孕症夫妇实施IUI治疗的 2 6 6个周期。根据不孕的原因分为不明原因组、免疫性不孕组、男性因素组、卵巢因素组及输卵管因素组。根据IUI方案分成自然周期 (NC)组、克罗米芬 (CC)组、尿促性腺素 +绒促性腺素 (HMG +HCG)组、CC +HMG+HCG组。比较各组间临床妊娠率。结果　每周期临床妊娠率为 10 .2 %。每例临床妊娠率为 13.3%。原发不孕组临床妊娠率明显高于继发不孕组 (17.6 %对 4 .5 % )。输卵管因素性不孕组临床妊娠率明显低于其他不孕组。不同IUI方案间的临床妊娠率无统计学差别。CC组的黄素化卵泡未破裂综合征 (LUF)发生率较NC组和HMG +HCG组明显增高。IUI治疗 2周期临床妊娠率明显高于治疗 1周期和 3周期及以上者。结论　宫腔内人工授精是治疗非输卵管因素不孕的一种有效方法 ,治疗时限以 3周期为宜

Clinical Analysis on Intrauterine Insemination in 266 Cycles

Objective To summary theClinical results ofIntrauter ine insemination of the 266 cycles.Methods Two hundred and sixty-six cycles in 203 couples undergoing intrauteine inseminatlon,in our hospital from June 1999 to April 2002,were retrospectively analyzed. A total of 203 couples weredivided into five groups according to the infetility reason:unexplained infertility,immune infertility,male factor,ovarian factor and tubal factor.A total of 266 cycles were divided into four groups according to the different plan of ovulatiln induction:nature cycles,clomiphene citrate(CC),HMG+HCG,CC个HMG+HCG.The pregnancy rate was compared.Results The pregnancy ratie was10.2% per cycle and 13.3% per patient.The gravidity rate of original infertility group was obviously higher than that of the secondary infertility group(17.6% versus 4.5%).The clinical pregnancey raty in the tubal factor group was obviously lower than others.There was no ststistlcs difference in clinical pregnancy rate among the different plans of ovlatiln induction.The LUF generant rate of CC group was obviously higher than NC group and HMG+HCG group.The clinical pregnancy rate by IUI for 2 periods was obviously higher than for 1 or 3 and above periods.Conclusion IUI is a very useful method of treating the sterility causing by non-salpinx factors,the best time limit is 3 periods.
