氯沙坦对老年原发性高血压病患者肾脏血流动力学的影响

摘    要:目的:观察氯沙坦对老年原发性高血压病左室肥厚患者肾脏血流动力学的影响.方法:应用彩色多普勒超声检测43例老年原发性高血压病患者治疗前后的肾动脉血流的变化值,测量指标包括肾动脉舒张期最低血流速度(Vdmin)、收缩期最大血流速度(Vsmax)、平均流速(Vm),计算肾血流量(Q)、肾阻力指数(RI)、搏动指数(PI)、Vsmax/Vdmin比值.结果:治疗后左肾动脉的RI、PI、Vsmax/Vdmin值均显著降低(P<0.01),右肾动脉的RI、PI及Vsmax/Vdmin值均降低(P<0.05).治疗后肾动脉血管壁厚度明显变薄;与治疗前比较差异有高度显著性(P<0.01),肾动脉血管内径治疗后增粗(P<0.05).结论:氯沙坦具有保护肾脏功能的作用.

Abstract:Objective: To observe the effect of Losartan on renal hemodynamics of the patients with essential hypertention complicating left ventricular hypertrophy. Methods: Renal blood flow of the patients was detected with color Doppler ultrasonography before and after treatment. Velocity diastolic minimal (V dmin ),velocity systolic maximal (V smax ) and Velocity mean (V m) were measured. Quantity blood flow (Q),Resistance Index (RI), Pulsation Index (PI) and V smax /V dmin were calculated. Results: RI,PI and V smax /V dmin were markedly lower in left renal ( P <0.01) and right renal after the treatment ( P <0.05). Thickness of renal arterial wall was significantly decreased ( P <0.01), diameter of renal arterial obviously increased ( P <0.05) after administation. Conclusion: Losartan may protect renal functions.
