摘    要:目的探讨不同影像介导的合理性、双切割系统活检针的作用以及对临床受益反应的影响。方法238例疑诊为恶性肿瘤的肺部病变患者分成两组,应用Biopince^TM full core切割针经皮肺活检,79例于DSA下进行,其余159例CT下进行。结果233例患者取得了满意的组织学标本,取材成功率为97.90%。DSA、CT组活检阳性率分别为89.33%、96.32%(P〈0.05)。两组病例共有91例患者经手术病理证实,DSA、CT组符合率分别为88.89%(24/29)、96.66%(60/62)(P〈0.05)。主要并发症为气胸和出血,DSA、CT组发生率分别为20.00%和12.27%(P〉0.05)。结论合理选择影像导向和活检针,可以提高取材准确性,减少创伤,从而提高诊断正确率,降低并发症发生率。

Abstract:Objective To evaluate the role of different imaged-guidlng, the function of "Cut & Trap" cannula system, and their clinical benefits for percutaneous lung biopsy. Methods All 238 cases suspected lung malignancy were divided into two groups: 79 cases underwent DSA-guided biopsy, 159 underwent CT-guided biopsy using "BiopinceTM full core" biopsy needle. Results Satisfied histological specimens were obtained in 233 cases. The success rate of biopsy was 97.90%, and the positive diagnostic rate was 89. 33% and 96. 32% in DSA-guided and in CT guided biopsy, respectively (P〈 0.05). Diagnosis of 91 cases were confirmed by surgery and pathology, and the accordance rate was 88.89% (24/29)in DSA guided, and 96.66% (60/62) in CT-guided (P〈0.05). Complications iricluded pneumothorax and bleeding, and the incidence of complications was 20.00% and 12.27% of DSA and CT guiding, respectively (P〈0. 05). Conclusion Reasonable chosing of imaged-guiding and biopsy needle will improve diagnostic accuracy and reduce complication.
