心房颤动患者凝血分子标志物的测定及其临床价值

目的：探讨心房颤动（房颤）与血浆凝血分子标志物的关系及其临床意义。方法：用酶联免疫双抗体夹心法分别测定房颤组、非房颤的心脏病患者（非房颤组）和健康人（对照组）血浆Ｄ－二聚体、血浆α－颗粒膜蛋白（ＧＭＰ－１４０）含量，同时用免疫浊度法分别测定各组血浆纤维蛋白原（Ｆｇ）浓度。对结果进行统计学分析。结果：房颤组Ｄ－二聚体、ＧＭＰ－１４０及Ｆｇ含量均显著高于正常对照组（Ｐ＜０．００１）。房颤组与非房颤组相比，Ｄ－二聚体和ＧＭＰ－１４０含量升高显著（Ｐ＜０．０５），但Ｆｇ差异不明显。另外还发现Ｄ－二聚体和ＧＭＰ－１４０水平在风心房颤与非风心房颤之间存在差异（Ｐ＜０．０５）。结论：房颤患者体内存在着凝血活性增强的因素，可能与其易发生血栓栓塞并发症有关。

Clinical value of detecting coagulation molecular markers in patients with atrial fibrillation

Objective: To explore the relationship between atrial fibrillation (AF) and coagulation molecular markers.Methods: 30 patients (groupⅠ) with AF;28 patients(groupⅡ) with various heart diseass in sinus rhythm; and 25 normal controls (groupⅢ) were included in this study. Plasma levels of fibrin D dimer and α granule external membrane protein (GMP 140) were measured using enzyme linked immunosorbent assay (ELISA) method;and plasma concentration of fibrinogen (Fg) was determined using immunoturbidity method.Results: plasma levels of D dimer,GMP 140,and Fg were significantly higher in (groupⅠ) thanin groupⅢ ( P <0.001).Except Fg ,lveles of D dimer and GMP 140 were significantly higer in group Ⅰ than in groupⅡ( P <0.05).In addition,patients with rhuematic AF had higher levels of D dimer and GMP 140 compared with nonrhuematic AF( P <0.05).Conclusion: The results demonstrated that patients with AF have an enhanced coagulability,which may be responsible for the development of thromboembolism.
