白细胞介素10在系统性红斑狼疮中的异常表达及其意义的初步研究

摘    要:目的:分析Th2型 细胞因子白细胞介素(IL)-10在系统性红斑狼疮(SLE)发生发展中的变化及其意义。方法:以37例符合美国风湿病学会(ARA)诊断标准的SLE患者为研究对象,与相匹配的正常人26名相对照,以ABC-ELISA法检测血清及细胞培养上清的IL-10水平,其中部分标本同时以荧光定量RT-PCR分析IL-10mRNA的表达。结果:SLE患者血清IL-10水平显著高于正常人(P<0.01),伴狼疮性肾炎者和抗双链DNA抗体阳性者其IL-10水平更高,活动期与缓解期患者相比,IL-10水平有所升高,但在统计学上差异无显著性;血清IL-10水平与外周血单个核细胞(PBMCs)中mRNA水平呈正相关性,相对于正常人而言,SLE患者的IL-10 mRNA亦呈高表达。体外培养结果显示,SLE患者PBMCs自发分泌或植物血凝素(PHA)诱生IL-10的水平均显著高于正常人(P<0.05)。结论:细胞因子IL-10的蛋白水平和mRNA转录水平,在SLE患者均明显升高,高水平的IL-10可能与狼疮性肾炎的姓有关。SLE患者PBMCs产生IL-10的能力异常增高。

Abstract:Objective:To evaluate the significance of abnormal expression of interleukin 10(IL 10) in systemic lupus erythematosus and the relationship between IL 10 aberrant expression and lupus predisposition, disease activity and prognosis. Methods:ABC ELISA was used to analyze IL 10 serum levels, and LightCycler fluorescent quantification system was used to validate the aberrant expression of IL 10 mRNA.Results:Either the serum IL 10 levels or IL 10 mRNA expression was significantly increased in patients with SLE compared with the control.There was a positive linear correlation between serum IL 10 and IL 10 mRNA expression in PBMCs. IL 10 levels measured in serum and in supernate of cultured PBMCs in SLE patients with positive anti dsDNA antibody or with lupus nephritis were significantly higher than those measured in patients with negative anti dsDNA antibody or without lupus nephritis respectively. Although IL 10 levels of active patients were higher than those of inactive ones, there was no significant difference in statistics; furthermore the IL 10 levels are not correlative to the SLEDAI. The IL 10 level in cultured PBMCs supernate of SLE patients, whether it was spontaneously secreted or after stimulation by PHA, was significantly higher than that in normal control. Conclusion:Abnormally high expression of IL 10 may contribute to the pathogenesis of SLE, especially to certain damages,such as lupus nephritis.
