影响子宫内膜癌预后的高危因素分析

目的:回顾性分析总结影响子宫内膜癌预后的高危因素。方法:对2000年12月~2004年12月中山大学附属第三医院、中山大学附属第二医院及中山大学附属肿瘤医院妇科收治的189例子宫内膜癌病例进行回顾性分析,进一步探讨病理类型、组织学分级、肌层浸润深度、淋巴结转移、手术病理分期、合并症与预后之间的关系。结果:手术病理分期、不同病理类型、组织学分级、有无肌层浸润和淋巴结转移中患者的生存曲线比较,结果均P<0.05,差异有统计学业意义;有无合并糖尿病、高血压或肥胖患者的生存曲线比较,结果均P>0.05,差异无统计学意义。COX回归多因素分析,组织学分级与肌层浸润深度对生存时间的影响差异有统计学意义(P<0.05)。结论:特殊病理类型、组织学低分化、深肌层浸润、淋巴结转移、期别晚是提示预后不良的高危因素。

Analysis on high risk factors of endometrial carcinoma prognosis

Objective:To summarize the high risk factors of endometrial carcinoma prognosis.Methods:189 cases of endometrial carcinoma from three hospitals were analyzed retrospectively from December 2000 to December 2004,the relationships between pathologic types,histologic grading,myometrial invasion depth,lymphatic metastasis,surgical-pathologic staging,complications and prognosis were explored.Results:There was significant difference in survival curves between different surgical-pathologic stagings,pathologic types,histologic gradings,different depths of myometrial invasion and lymphatic metastasis or not(P<0.05).There was no difference in survival curves between cases with diabetes or not,hypertension or not and obesity or not(P>0.05).Multivariate analysis indicated that histologic grading and depth of myometrial invasion were independent high risk factors for prognosis of endometrial carcinoma(P<0.05).Conclusion:Special pathologic types,low histological grading,deep myometrial invasion,lymphatic metastasis,late pathologic staging are high risk factors of endometrial carcinoma prognosis.
