| 摘 要： | 目的:探讨鞘内不同剂量布比卡因复合舒芬太尼腰硬联合阻滞在分娩镇痛中的作用。方法:选择初产妇150例,随机分五组:C组、B1组、B2组、B3组、B4组,采用双盲对照观察,各组均为蛛网膜下腔给药后硬膜外留管产妇自控镇痛。C组鞘内药物仅为舒芬太尼2.5μg;B1～B4组为布比卡因复合舒芬太尼2.5μg,布比卡因剂量分别为0.5 mg、1.0 mg、1.5mg、2.0 mg。各组硬膜外维持用药相同。比较各组鞘内给药后5 min、10 min、15 min、30 min的视觉模拟评分、镇痛维持时间、运动阻滞情况、各产程时间、出血量、新生儿1 min和5 min Apagr评分。结果:起效时间C组>B1组>B2组>B3组>B4组,维持时间C组0.05)。结论:舒芬太尼2.5μg混合0.5～2.0 mg布比卡因的鞘内注射均可提供安全、有效的分娩镇痛效果,其中鞘内舒芬太尼2.5μg混合1.0 mg布比卡因的分娩镇痛效果最佳。

Effect of different doses of intrathecal bupivacaine with sufentanil on labor analgesia |

| Abstract: | Objective:To explore the effect of different doses of intrathecal bupivacaine with sufentanil on labor analgesia.Methods:150 nulliparous women were selected and divided into 5 groups randomly:Group C,Group B1,Group B2,Group B3,Group B4.All of the women were given spinal injection.Group C was only received sufentanil 2.5μg.Group B1~B4 were received different doses of bupivacaine from 0.5~2.0 mg and sufentanil 2.5μg.VAS was assessed on 5,10,15,30 minutes after injection.The onset time of block,duration of analgesia,motor block,duration of labor,bleeding volume,Apgar score(l minute and 5 minutes) and probable side effects were compared between 5 groups.Results:Group C was the latest onset group in the study,while Group C
