爆破模式在硬核白内障超声乳化术中的应用

目的观察比较在硬核白内障超声乳化术中应用爆破模式与连续模式的疗效。方法将69例硬核白内障患者(69只眼)随机分为连续能量模式组(A组31例)和爆破能量模式组(B组38例),记录两组术中使用的实际超声乳化时间、平均超声能量,比较术后第1,3,7天术眼最佳矫正视力,术后角膜水肿情况、手术并发症等。结果爆破组术中实际超声乳化时间、平均超声能量均较连续模式组明显少,两组相比差异有统计学意义(P<0.05),术中后囊膜破裂,两组差别无统计学意义(P>0.05);爆破组术后第1,3天最佳矫正视力明显高于连续超声模式组,差别有统计学意义(P<0.05),术后角膜水肿眼数爆破模式组明显低于连续模式组,两组差别亦有统计学意义(P<0.05)。结论应用爆破能量模式行硬核白内障超声乳化术,可明显减少超声能量和时间,减少术后早期角膜水肿,更具有安全性和有效性,值得在临床推广使用。

Application of Burst Mode in Phacoemulsification of Hard Nucleus Cataract

Objective To investigate and compare the therapeutic effect of burst mode and continuous mode in the phacoemulsification of hard nucleus cataract..Methods Sixty cases of hard nuclear cataract(69 eyes) were randomly divided into continuous energy mode group(31 cases) and burst mode group(38 cases).The actual time and average energy of phacoemulsification in both groups were recorded.The best correction vision of operated eyes,edema level of cornea and surgery complications was compared on 1,3 and 7 days after operation.Results The time and average energy in the burst mode group were much less than that in the continuous mode group,and the difference had statistics significance(P<0.05);The difference in the rate of posterior capsule rupture in the operation was not significant between the two groups(P>0.05);The postoperative best corrected visual acuity on day 1 and 3 in the burst mode group was better than that in the continuous energy mode group(P<0.05);The incidence of corneal edema in the burst mode group was obvious lower than that in the continuous mode group(P<0.05).Conclusion The burst mode group in phacoemulsification of hard nucleus cataract can obviously decrease the energy and time,reduce the incidence of corneal edema,is more safe and effective in clinical use.
