摘 要: | 目的探究全血粘度对健康成人脑血流速度影响。方法检测100例健康成年人血细胞比容(Hct)及血浆纤维蛋白原(Fg)水平,且在当天完成经颅多普勒(TCD)检查,同时记录各检查动脉平均血流速度(Vm)。并分别分析两者与平均血流速度相关性。结果 Hct与大脑中动脉(MCA,r=-0.39,P<0.001)、颈内动脉虹吸部(SCA,r=-0.16,P<0.05)、颈内动脉颅外段(ICAex,r=-0.15,P<0.01)、椎动脉(VA,r=-0.23,P<0.01)及基底动脉(BA,r=-0.35,P<0.001)平均血流速度具有显著性负相关关系。而Fg仅与SCA平均血流速度具有负相关关系(r=-0.19,P<0.05)。低血细胞比容组(Hct≤40%)的MCA、VA、BA的Vm较高血细胞比容组(Hct>40%)明显增高。结论建议TCD正常脑血流速度参考值应考虑Hct因素并根据被检测者的Hct水平进行适当校正,尤其是对MCA、VA、BA等血管。当被检测者的脑血流速度轻度增快时应考虑Hct因素,而不仅仅只考虑到血管本身的病变如血管狭窄。

Abstract: | Objective To investigate the correlation between whole blood viscosity and cerebral blood flow velocity in the healthy adults by transcranial Doppler(TCD).Methods Hematocrit and fibrinogen concentrations were measured in 100 healthy adults,and the TCD test was performed on the same day.The mean velocities of all major cerebral arteries were recorded and analyzed.For the purposes of this study,we averaged the mean velocities from the bilateral measurements.Results The hematocrit was significantly and inversely correlated with the mean velocities of MCA(r=-0.39,P<0.001),SCA(r=-0.16,P<0.05),ICAex(r=-0.15,P<0.01),VA(r=-0.23,P<0.01) and BA(r=-0.35,P<0.001).However,the fibrinogen was only inversely correlated with SCA(r=-0.19,P<0.05).Besides,the subjects with low hematocrit(Hct≤40%) showed significantly higher mean velocities of the MCA,VA,and BA.Conclusion The hematocrit can significantly affect TCD reference values in healthy adults and should be taken into account or adjusted in TCD applications,particularly for the MCA,VA,and BA measurement.So when we detect a slight fast velocity of these arteries,Hct factor should be considered,not only take into account the artery disease itself,such as stenosis. |
