survivin蛋白在乳腺癌中表达水平及与cerbB-2关系的Meta分析

目的系统评价存活蛋白(survivin)表达水平在乳腺癌(breast cancer)中表达水平及与人表皮生长因子受体2(cerbB-2)关系。方法计算机检索Cochrane Libtary(2011年第1期),PubMed、CNKI等数据库,并辅以手工检索,按照纳入与排除标准选择病例对照试验,评价质量及提取资料后采用Stata 11.0软件对数据库进行系统评价。结果共纳入16项研究,其中1330例乳腺癌患者,325例正常对照。Meta分析结果显示:survivin在乳腺癌组及正常对照组[比值比(odds ratio,OR)=51.34,95%可信区间(confidence interval,CI)=29.85-88.30]的表达差异具有统计学意义(P<0.05)。进一步生存分析显示,survivin阳性表达的乳腺癌患者5年生存率明显低于阴性者(OR=8.25,95%CI=1.89-35.96,P<0.05)。Spearman等级相关分析显示,survivin与cerbB-2蛋白阳性率在乳腺癌中表达具有相关性(r=0.275,P<0.05)。结论 survivin在乳腺癌中高表达,降低了乳腺癌患者5年生存率,同时survivin与cerbB-2在乳腺癌表达也有一定的相关性。

Expression of Survivin in Breast Carcinoma and its Relationship with CerbB-2

Objective To investigate the expression of survivin protein in breast cancer and its relationship with cerbB-2 by a meta analysis.Methods The published studies were searched in the Cochrane Library(issue 1,2011),Pubmed and CNKI databases,and other relevant journals were also hand searched to identify all the relevant case-control trials.Then the quality of the included trials was assessed and meta-analysis was performed by Stata 11.0 software.Results A total of 16 studies were recruited.As for the positive rate of survivin expression,significant differences were tested between esophageal cancer vs.normal esophageal tissues(OR=51.34,95%CI=29.85-88.30,P<0.05).Moreover,the overall survival time was significantly shorter in patients with survivin expression than that with absent survivin expression(OR=8.25,95%CI=1.89-35.96,P<0.05).survivin expression was positively correlated with cerbB-2(r=0.275,P<0.005).Conclusion The survivin expression can increase the malignant behaviors of breast cancer,and meanwhile substantial correlation is confirmed between survivin and cerbB-2.
