腹腔镜与开腹手术治疗急性阑尾炎的临床对比研究

目的对比分析腹腔镜和传统开腹阑尾切除术治疗急性阑尾炎的疗效。方法回顾分析2009年4月-2011年7月蚌医二附院收治的急性阑尾炎患者132例的临床资料,其中传统开腹手术68例,腹腔镜手术64例。比较两种术式的手术时间、术中出血量、下床活动时间、止痛药使用率、切口感染率、术后并发症、平均住院时间及综合费用。结果腹腔镜组术中出血量、肛门排气时间、切口感染率、平均住院时间、粘连性肠梗阻、镇痛剂应用例数与开腹手术组的差异有统计学意义(P<0.05),两组手术时间差异无统计学意义(P>0.05),LA组住院费用明显高于OA(P<0.05)。结论腹腔镜比传统开腹手术治疗急性阑尾炎有创伤小,康复快,住院时间短,并发症少,疗效好等优点,是治疗急性阑尾炎较理想的手术方式。

Clinical Comparative Study of Laparoscopy versus Open Surgery for Acute Appendicitis

Objective To compare and evaluate the effect of laparoscopic and traditional open surgery in the treatment of acute appendicitis.Methods From Mar.2009 to July.2011,the clinical data of 132 cases with acute appendicitis were analyzed retrospectively.Among them,64 cases were treated with laparoscopic appendectomy and 68 cases with open surgery.The duration of operation,peri-operative blood lost,active time,using of antalgesic,surgical wound infection rates,complications,hospital stays and cost of hospitalizing patients were studied.Results The difference in peri-operative blood lost,anal exhaust time,mean hospital stays,adhesive intestinal obstruction and using of antalgesic were significant between the two groups(P<0.05).There was no significant difference in the duration of operation(P>0.05).The cost of hospitalizing patients in laparoscopic appendectomy group was higher than that in open surgery group(P<0.05).Conclusion The laparoscopic appendectomy,with the advantages of minimal invasion,early recovery,few complications and short hospitalization time,is a more popular choice for the patients with acute appendicitis.
