射频消融治疗变应性鼻炎的临床疗效观察

目的观察低温等离子射频消融治疗非尘螨引起的变应性鼻炎的临床疗效。方法回顾性分析2008年10月-2009年10月耳鼻咽喉科收治的60例变应性鼻炎患者,皮肤点刺实验显示尘螨阳性小于(++),近1周内未服用任何抗过敏药或感冒药的患者,在鼻内镜直视下于双侧鼻丘,鼻中隔前上方筛前神经走行处,中鼻甲1～2点,下鼻甲2～3点行低温等离子射频消融术,术后1个月和1年评定疗效。结果术后1个月随访,24例症状、体征改善率≥66%,为显效,占总数40%,36例症状、体征改善率在65%～26%,为有效,占总数60%,总有效率为100%。术后1年随访,21例症状、体征改善率≥66%,为显效,占总数35%,30例症状、体征改善率在65%～26%,为有效,占总数50%,其余9例症状、体征改善率≤25%,为无效,占总数15%,总有效率为85%。结论低温等离子射频消融术是治疗非尘螨引起的变应性鼻炎的一种简便、安全、实用的治疗方法,值得临床推广使用。

Clinical Curative Effect of Temperature-controlled Radiofrequency Ablation on Allergic Rhinitis

Objective To observe the clinical curative effect of low temperature radiofrequency ablation(RFA) in treating allergic rhinitis which was not caused by dust mites.Methods Sixty patients of allergic rhinitis took skin prick test which showed positive less than(++) for dust mites from October 2008 to October 2009.The patients were not taking any allergy or cold medicine in the next week.They were treated by RFA at bilateral agger nasi,nasal septum before the top of the ethmoid nerve,1 to 2 point of turbinate,2 to 3 point of inferior turbinate with the help of nasal endoscope.Its curative effects were evaluated as well one month and a year after operation.Results After one month follow-up,24 patients with symptoms and signs of improvement rate ≥66%,markedly effective,40% of the total;36 patients with symptoms and signs of improvement rate between 65% and 26% for the effective,accounting for 60%;the total efficiency ratio was 100%.After one year follow-up,21 patients with symptoms and signs of improvement rate ≥66%,markedly effective,35% of the total;30 patients with symptoms and signs of improvement rate between 65% and 26% for the effective,accounting for 50%;the remaining nine cases with symptoms and signs of improvement rate ≤25%,as invalid,15% of the total,with a total effective rate was 85%.Conclusion The low temperature radiofrequency ablation is worth to be used in clinical for its simple,safety and practical in treating allergic rhinitis.
