摘    要:目的探讨抗心磷脂抗体(ACA)与脑出血关系,为脑出血早期病情变化机制和脑出血免疫治疗提供新的观点。方法应用酶联免疫吸附(ELISA)法对101例健康人,271例脑出血病人发病1~3 h、24 h、14 d检测ACA(IgG、IgA、IgM)水平。结果脑出血组ACA阳性率(43.91%)高于健康组(12.87%),差异有统计学意义(P<0.01)。脑出血组随着病情加重ACA阳性率逐渐增高,随发病时间的延长ACA阳性率逐渐减少,差异有统计学意义(P<0.05,P<0.01),与IgA、IgM相比IgG更为敏感(P<0.01)。结论 ACA与脑出血病情程度呈正相关,ACA可作为评价脑出血病情程度的重要指标,其中IgG ACA最为敏感,免疫机制可能在脑出血早期病理变化机制中扮演重要角色。

The Application of Anticardiolipin Antibody in Cerebral Hemorrhage

Abstract:Objective To study the relationship between anticardiolipin antibody(ACA) and cerebral hemorrhage and to provide the new perspectives for the mechanisms of early changes in cerebral hemorrhage and immunotherapy treatment for cerebral hemorrhage.Methods The positive rate of serum ACA was determined by enzyme linked immunosorbent assay(ELISA) in 101 health people and 271 patients with cerebral hemorrhage at 1-3 h,24 h and 14 d after the onset of the disease.Results The positive rate of serum ACA in cerebral hemorrhage group(43.91%) was significantly higher than health people group(12.87%) and difference was statistically significant(P<0.01).With the progression of disease,the positive rate of ACA in cerebral hemorrhage increased gradually,and difference was statistically significant(P<0.05,P<0.01).With the onset extension of disease,the positive rate of ACA gradually decreased and difference was statistically significant(P<0.01).IgG compared to IgA and IgM was more sensitive(P<0.01).Conclusion There was a positive correlation for ACA and cerebral hemorrhage.To provide an effective indicator for evaluation of severity of cerebral hemorrhage,which IgG ACA was the most sensitive.The immune mechanism may play an important role in the early pathological changes of cerebral hemorrhage.
