食管中段鳞癌左、右胸两种入路疗效的比较

目的从食管中段鳞癌手术后的远期生存率方面评价经左胸与经右胸两种手术入路的疗效。方法对244例经手术治疗并有随访资料的病例进行统计分析,比较经左胸与经右胸两种手术入路患者1年、3年、5年生存率差异。结果两组患者1年、3年、5年生存率:经左胸组分别为79.4%(81/102)、48%(49/102)、29.4%(30/102);经右胸组分别为82.4%(117/142)、57.6%(81/142)、29.6%(42/142),组间行χ2检验,对应P值分别为P=0.5107,P=0.6025,P=0.9777,均大于0.05,差异无统计学意义。结论两种手术入路组患者的1年、3年、5年生存率差异无统计学意义,但手术入路选择应个体化,对有上纵隔和腹腔淋巴结明显肿大的患者,应采用经右胸入路。

Comparison of Surgical Resection of Left and Right Transthoracic Approaches Formiddle Thoracic Esophageal Squamous Cell Carcinoma

Objective To explore the curative effects of left and right transthoracic approaches for middle thoracic esophageal squamous cell carcinoma by evaluating the postoperative long-term survival rate.Methods 244 cases of resected squamous cell carcinoma of the middle thoracic esophagus which were followed up were reviewed in Jiaozuo People’s Hospital from 2000 to 2008.The survival rate of the two groups was compared.Results The one-,3-and 5-year survival rate was 79.4%,48.0%,29.4% in left approach group and 82.4%,57.6%,29.6% in right approach group.The difference in 1-,3-and 5-year survival rate had no significant between the two groups(P=0.5107,P=0.6025,P=0.9777 and P>0.05).Conclusion There was no significant difference in the survival rate between two surgical approaches,but the choice of the surgical approaches should be individualized.Esophagectomy of right transthoracic approach should be used when superior mediastinum and abdominal cavity-Lymph node enlargement was obvious.
